Malvern Eyecare - Permanently Closed

Chemist Warehouse Optometry – Malvern Eyecare at 120 Glenferrie Road permanently closed on 21 August 2026. We sincerely thank our patients and the Malvern community for allowing us to provide their eye care.

Your patient records remain secure
Unfortunately, we do not have another nearby location able to take over your ongoing care. You are free to choose any optometrist or other healthcare provider. All Malvern patient records continue to be securely retained by Optometrist Warehouse Pty Ltd, trading as Chemist Warehouse Optometry. 

You may request:
•    A copy of your patient record for yourself; or
•    Secure transfer of the relevant record to your chosen healthcare provider.

Requesting your records
Please send an email to malvern@optometristwarehouse.com.au.
Use the subject line: "Patient record request" and include the following information in your email:
•    Your full name;
•    Your date of birth;
•    Your current telephone number; and
•    If requesting a transfer, the name and contact details of your chosen provider.

Please do not attach identity documents or detailed health information to your initial email. We will contact you to verify your identity and authority and arrange secure delivery.

No immediate action is required if you do not currently need your record. It will remain securely retained and accessible in accordance with applicable record-retention requirements.

If you experience sudden vision loss, significant eye pain or another urgent eye problem, please seek prompt care from a local optometrist, GP or hospital emergency department. Call 000 in an emergency.
